We are the Health Equity Alliance of Long Island (HEALI), a Social Care Network (SCN) connecting community-based organizations and partners to support equitable health and life outcomes for our neighbors.
To get help and confirm eligibility for services, contact a HEALI Navigator for assistance at or 516-505-4434. Our Navigators will also ask you some questions to help identify where you might need some extra support.

To access Medicaid-reimbursable services, you will need to complete a screen to better understand your needs and see what you qualify for. We’re here to help! Please contact our Navigators at or 516-505-4434, and we’d be happy to assist you through the process.

HEALI brings together community-based organizations (CBOs) and partners of all sizes and specialties into a single network, integrating social care with health care. This powerful network helps address disparities by improving access to nutritious meals, housing supports, transportation, and other social services that have a significant impact on an individual’s health.
The New York State Department of Health leads the nation with the introduction of SCNs to manage the coordination of social care and partner with healthcare to improve health outcomes. HEALI, Long Island’s designated SCN, leads the region to support a healthier Long Island.
