HEALI brings together community-based organizations (CBOs) and partners of all sizes and specialties into a single network, integrating social care with health care. This powerful network helps address disparities by improving access to nutritious meals, housing supports, transportation, and other social services that have a significant impact on an individual’s health.
The New York State Department of Health leads the nation with the introduction of SCNs to manage the coordination of social care and partner with healthcare to improve health outcomes. HEALI, Long Island’s designated SCN, leads the region to support a healthier Long Island.
