Who we are

We are the Health Equity Alliance of Long Island (HEALI), a Social Care Network (SCN) connecting community-based organizations and partners to support equitable health and life outcomes for our neighbors.

HEALI was founded as a coalition within the Health and Welfare Council of Long Island (HWCLI) with support from the New York State Department of Health as part of the Delivery System Reform Incentive Payment (DSRIP), a program that uses federal funds to help improve healthcare delivery systems. In 2024, HEALI was named Long Island’s SCN, with HWCLI named as lead entity.

HEALI’s members are Community Based Organizations (CBOs), healthcare providers, health related service providers, and HWCLI, and the network is governed by an elected and independent governing body. As the lead entity, HWCLI contracts on behalf of the HEALI SCN with government, service providers, Managed Care Organizations (MCOs), and vendors to operate the network and provide services.

How can I get involved?

CBOs, healthcare providers, and Health Related Service Need (HRSN) providers serving Nassau and Suffolk counties can become a member of the HEALI SCN.

CBOs that are members of the HEALI SCN are eligible for capacity building funding to engage in the network.

CBOs, healthcare providers, and HRSN providers that are members of the HEALI SCN can contract with HWCLI to provide SCN services to Medicaid members. The HEALI SCN lives within a larger social care ecosystem on Long Island that includes Managed Care Organizations (MCOs), health systems, government, child and family supports, and other partners. All are welcome to participate in HEALI SCN monthly network meetings and HEALI SCN committees.

Our mission

Our mission is to engage health and human service agencies, community members, and other stakeholders to ensure equitable health and life outcomes for all Long Islanders through cross-sector partnerships, innovative funding strategies, and improvements in the health & human service delivery system. We will use our collective power to influence public policy and investment decisions by funders.

Our vision is to build and provide an integrated, holistic, culturally and linguistically responsive health and human service delivery system that results in equitable health and life outcomes for all Long Islanders.

Meet the team

Meet the governing body​

As the lead entity for the HEALI SCN, HWCLI is responsible for establishing and maintaining a governing body that reflects and understands the unique needs of the region and effectively coordinates among other stakeholders in the region. This governing body will be comprised of a wide array of stakeholders in the region, including (but not limited to) CBOs delivering Health Related Social Needs (HRSN) services, providers, advocacy organizations, members, and other community members who are representative of the communities served by the SCN.

The Governing Body provides a structure by which the SCN Lead Entity receives feedback from Network members and other community partners, aligns on shared priorities and strategic goals, and collaboratively makes and approves important operational decisions.
