HEALI is working with healthcare offices across Long Island to offer a new program to help people living with diabetes
or prediabetes who have Medicaid health insurance get free healthy food and nutrition help at home or in their community.
Good health isn’t just about going to the doctor. It’s also about having enough healthy food to take care of yourself.
This program is here to make that easier.

How the Program Works
1. Simple Screening
During your visit today, we may ask you some simple questions. These questions help us understand if you need extra help, including help with food.
2. Help with Food
If you are interested and are eligible, HEALI, which is supported by Medicaid and the NYS Department of Health, works directly with local organizations in your community who can provide:
- Healthy meals delivered to your home
- Food boxes delivered to your home or grocery vouchers
- Cooking supplies
- Help learning how to cook healthy meals

- It will not your change your Medicaid benefits
